PAC Medical Hardship Waiver (Redshirt) Application Checklist

 

   The Student-athlete should submit the following in the application process for a medical hardship waiver (redshirt) waiver:

 

1.      A letter to the PAC medical hardship waiver committee seeking the waiver that includes:

The season in which the injury occurred

A brief synopsis of the injury/why the waiver is being sought

The result of the injury that caused the applicant to seek the waiver

Complete mailing address and current contact information for the applicant

 

2.      Complete and appropriate records from the institution’s certified Athletic Trainer

 

3.      Complete and appropriate documentation from the PHYSICIAN that treated the injury for the athlete. (Verification of dates of appointments is crucial)

 

4.      Complete copies of all individual and team statistics for the season in which the waiver is being sought.

 

5.      A team schedule for the season in which the waiver is being sought

                                                                                  

 

   The PAC medical hardship waiver committee will review applications for medical hardship waivers at the PAC Athletic Director’s council meetings which occur three times during the Academic year. It is the applicant’s responsibility to make sure the completed file is secured by a member of the PAC medical hardship waiver committee at least 72 hours prior to the AD council meeting dates. Files that are incomplete or tardy will be reviewed at the next or following AD council meeting. The PAC medical hardship waiver committee chair will notify the applicant in writing as to the decision within a week of the PAC AD council meeting in which the completed file is reviewed.  Only in rare or extenuating circumstances will a medical hardship waiver application file be reviewed by the committee outside of the PAC AD council meetings. Should a circumstance like this exist, those reasons should be clearly explained in the athlete’s letter of application. IT IS STRONGLY RECOMMENDED BY THE COMMITTEE THAT student-athletes seeking a medical hardship waiver begin the application process AS SOON AS POSSIBLE with respect to the season in which the injury occurred. It has been experienced by the committee that the longer the wait to seek the medical hardship waiver, the greater the chance that incomplete or inaccurate files appear.
